There is a large parking lot (free of charge) on either size of the building.
Bring Insurance Cards, Copay (if applicable), Picture ID, Current List of Medications & Drug Allergies, recent test results and lab results.
For new patients, there is a packet that can be filled out online. For returning patients, 2 forms are needed every time you visit (Symptom sheet & update medication sheet).
